Before your first visit, you need to complete paperwork in your portal account. The photo below shows the 3 areas where you will need to click:
- The orange button to complete CHADIS questionnaires
- The orange button to complete pre-checkin
- The messaging button. Once you click on the messaging button, select “DBP Policy Acknowledgement Form” and click “start message” to electronically sign our office policies.

Cold and flu season is upon us! 🦠 Fever is an important mechanism for fighting infection, and monitoring your child’s temperature is a great way to determine whether your child needs more care. Contact us immediately if:
🤒 Your baby is under 2 months old and has a fever of 100.4F or greater
😷 Your child has a consistent fever above 104F

Based on the current COVID-19 activity rates in our area, in-office masking is not required for parents and patients at this time. We are still sanitizing equipment and patient rooms between visits to make our office safe for patients, families, and staff.
