Paperwork instructions for new patients

Before your first visit, you need to complete paperwork in your portal account. The photo below shows the 3 areas where you will need to click:

  • The orange button to complete CHADIS questionnaires
  • The orange button to complete pre-checkin
  • The messaging button. Once you click on the messaging button, select “DBP Policy Acknowledgement Form” and click “start message” to electronically sign our office policies.

The orange CHADIS and pre-checkin buttons will appear 7 days before your appointment. If you have any questions or need to reset your password, please call or text the office for assistance at 915-280-9267!
